HCD is a national company with a local feel and you can use our services regardless of which local authority you come under.
At HCD we place great importance on making things as easy as possible for you. Instead of you having to speak to lots of different people about your support services we can be the common link between all professionals involved and coordinate everything for you, if that’s what you choose – we will do as much or as little as you like.
At HCD we have years of experience working with other professionals to design, develop and run care and support services. We fully understand and embrace the fact that no two people are the same or want services in the same way – we make sure that the support services you get are what you have chosen.
HCD has close working partnerships with local authorities, PCTs, social workers, and case management companies around the country who refer individuals to us for our wide variety of services. We share a common goal to help people live independently within their local communities.
HCD accept referrals from local authorities for anyone who requires care and support, including older people, children and young people, people with learning disabilities, people with mental health issues and people with physical disabilities and/or sensory impairments.
If you choose to use HCD’s services we will happily talk to your social worker on your behalf to help move things forward and get you the care and support you choose, in the way you want.
