HomeCareDirect are looking to recruit a new member to join our dedicated team at our Head Office in Chesterfield

Independent Living Advisor

Overview of the role:

To help individuals to live an independent life in line with their care and support plan. Your role is to ensure that the people who use the HomeCareDirect service are supported to achieve their goals and aspirations whilst making a success out of directing their own support at home.

You will liaise with the independent living team manager and other departments within HomeCareDirect to ensure that queries from individuals, families and care professionals are dealt with in an efficient and timely manner so that your support packages run smoothly.

  • Good IT Skills Edexcel qualficiation prefered or similar
  • Customer service experience prefered / good communication skills essential
  • Experience working in the health or social care industry
  • Transport prefered due to location of office

General duties:

  • Help the individuals and families you support to live independently and direct their own support, solving problems for them with a pro-active ‘can-do’ attitude and approach 
  • Accept responsibility for your client case load as dictated by the Independent Living Team manager 
  • Ensure Care Quality Commission regulations are adhered to in order to keep the client safe
  • Work closely with the Independent Living Nurses to ensure your support packages run smoothly 
  • Read log notes monthly and report and action any issues
  • Check medication administration records and report on/action any issues 
  • Ensure the care plan and risk assessments for your clients are up to date and are on file
  • Ensure that client reviews and personal assistant supervisions are booked and completed
  • Ensure all hours are covered that are outlined in the support plan, if possible.
  • Where applicable, ensure that care shifts within your support packages are covered 3 weeks in advance
  • Coordinate the actions created through individual client action plans. Facilitate and coordinate these actions and ensure that they are up-to-date at all times. Actions may be clinical or goal orientated
  • Liaise with all professionals and families involved in your support packages and carry out courtesy calls to clients and personal assistants
  • Build up a relationship with external funders and commissioners for your allocated area
  • Work with the HR department to ensure personal assistant working patterns are in line with the working time directive and HomeCareDirect policy
  • Work closely with the HR department to create personalised adverts for personal assistants within your allocated support packages, and to ensure that personal assistants are recruited in a timely fashion
  • Work closely with the training department to ensure that all personal assistants within your support packages are fully trained
  • Be on a rota set by your manager to cover the HomeCareDirect 24 hour support line
  • Be available at month end and on the 1st of every month to ensure your clients support hours are inputted into the system. It is your responsibility to ensure your hours are complete. Only your manager can authorise leave at these time. 
  • Where applicable, ensure your clients personal budget is met and full funds are utilised to enhance the clients health and well-being
  • Ensure tasks set by the Independent Living Team manager are completed in a timely manner
  • Ensure the HomeCareDirect care plan, risk assessments (and external care plans and risk assessments) are coordinated through the action plan 
  • Be available to undertake visits if your manager requests where reasonably practicable
  • Ensure all additional charges and Invoices notes are up-to-date and passed to the accounts department
  • Work with the accounts department to ensure invoice and payroll deadlines are met.
  • Keep your knowledge around current legislation and regulations surrounding the care industry up to date, whilst understanding your allocated funding area


Salary Range :- Starting  @ £16,700 – £18,700. Competency is achieved through module based learning, one to one training and external courses.

Further increase of salary after 12 months once competent working though a second level of development.

Hours Of Work:

Monday – Friday 9am – 5pm – 37.5 hours per week 

Where required you may be asked to work on days and times other than Monday – Friday 9am – 5pm including public holidays.
